ABSTRACT

COVID-19 has required a transformation of skill sets in counseling - what was previously done in face-to-face environments is now done in telehealth settings. Telehealth communication is evolving. As instructors and supervisors respond to this need, changes in training may impact interns as they learn to express empathy for clients whose nonverbal responses, at the very least, may be less evident in a telecommunication environment. Empathy has evolved from responding and relating to a client to a more involved, multidimensional skill that requires the perception of the various dimensions of the person including biological, psychosocial, genetic, and cultural components. This empathy is more than just a response;it is a commitment to understanding through the integration of these components within expressive communication. How can instructors best demonstrate concepts in a counseling relationship that is bound by distance and telecommunication? This chapter explores the definition, development, and use of empathy and introduces the concept of "cyber-empathy" as exercised in the training of counseling interns, as well as some of the potential obstacles that may be encountered in training and expression of empathy in this post-COVID era. Cyber-empathy expands and defines the work of training and supervising this new cohort of counseling professionals. © Springer Nature Switzerland AG 2021. All rights reserved.

ABSTRACT

Schools have become increasingly influenced by what has been called the Social Internet of Things (SIoT). As many schools transitioned to online teaching and learning in 2020 because of the COVID-19 pandemic, there is a need to analyze data on technology adoption in schools for changes and continuity since the inception of increasingly popular smartphones since 2007 that has affected videoconferencing, blended learning, and the flipped classroom concept. In this study, videoconference technology is the main factor analyzed in relation to IoT and schools in which handheld devices are considered gateways to further integration of schools into an IoT framework. The researchers analyze a selection of a 2009 dataset and a 2019 dataset from the National Center for Education Statistics (United States) to discuss teachers' perceptions of technology, especially the use of handheld devices and videoconference technology. Recommendations for educator preparation programs (EPPs) are made based on the discussion of the results of this analysis in context with the events of 2020 for why EPPs could emphasize flipped classroom design models within an SIoT framework. © 2023 IEEE.

ABSTRACT

INTRODUCTION AND OBJECTIVE: Telemedicine has gained wide acceptance during the COVID-19 pandemic. Worldwide lockdowns made this interface an indispensable one for patient care. Recent evidence suggests that urology patients were receptive to telemedicine;however, no systematic review has been done to date on andrology patients in particular and their perception of telemedicine. METHOD(S): Three electronic databases: PubMed, Scopus, and Web of Science, were searched from their inception until June 2022 for relevant articles. Two independent teams reviewed s and extracted data from the selected manuscripts. A meta-analysis was completed in line with PRISMA 2020 and AMSTAR Guidelines. For our study, we limited telemedicine to communication through videoconferencing or telephone encounters between patients and medical professionals. Positive response to telemedicine was defined as patients "wishing for telemedicine consultation", "preferring telemedicine over in person", "accepting the current telemedicine arrangement", "having needs addressed with teleconsultation", or "willingness to do a teleconsultation". RESULT(S): Of the 1128 retrieved s, 56 underwent fulltext review and 12 were included in the final analysis, comprising a total cohort of >4021 cases. All 8 studies that assessed perception of andrology patients to telemedicine showed that most patients had "positive perception". Telemedicine was shown to save an average cost of $149-252/patient. Pooled analyses of positive response to telemedicine were 68.7% (95% CI = 49.4-83.1%, p=0.057) and of patients who recommended telemedicine were 65.1% (95% CI = 18.4-93.9%, p=0.577). 84.6% pooled proportion for recommending video compared to 38.9 % pooled proportion for recommending telephone practice, p=0.035. In the telephone-only encounters, 27.1% of patients preferred in-person visits as security and privacy of any mode of telecommunication were of concern. CONCLUSION(S): Most patients have a positive perception of telemedicine, particularly with videoconferencing and less so with telephone visits. Telemedicine will plausibly continue to be integral in andrology practice.

The study of food products to determine the level of creatine in them is an actual task, taking into account the need for this substance for patients with Covid'19 and spinal muscular atrophy. The purpose of our research is to develop a mobile information system for determining the level of creatine in food products. The developed method for determining the level of creatine in food products by the user and the method for determining the level of creatine in food products using a mobile information system provide the user with the opportunity to quickly, conveniently, cheaply and effectively assess the presence and level of creatine in any food products, on the basis of which to build a rational diet from the point vision of body saturation with creatine. The proposed mobile information system for determining the level of creatine in food products provides convenience, low-cost, celerity, miniaturization and automation for measurement of concentration of creatine in any food products. The conclusion obtained from the system regarding the presence and level of creatine in this or that food product is useful and extremely important when preparing the diet of patients, especially patients with Covid'19 and/or spinal muscular atrophy. The proposed approach and mobile information system for determining the level of creatine in food products can be used not only for drawing up the diet of patients, especially patients with Covid'19 and/or spinal muscular atrophy, from the point vision of body saturation with creatine, but also for example, to check the quality of meat products. © 2023 Copyright for this paper by its authors.

ABSTRACT

Case Report: West Nile Virus (WNV) was first isolated from the West Nile district of Northern Uganda in 1937, but was first detected in the United States well over half a century later in 1999. The arthropod-borne virus has since persisted, with 2,401 cases reported to the CDC on average annually. The infection typically causes a nonspecific acute systemic febrile illness with occasional gastrointestinal and skin manifestations;however, in less than 1% of infected patients, it can cause severe and potentially fatal neuroinvasive disease, presenting as meningitis, encephalitis or acute flaccid paralysis. Immunosuppression is one of the risk factors associated with the development of neuroinvasive disease, and chemotherapy thus places patients at risk. Uterine leiomyosarcoma is a rare gynecological malignancy. Palliative chemotherapy is common in late stage disease, but may predispose patients to conditions that present as neutropenic fever, leading to a diagnostic conundrum. This is the first case report where patient with neutropenic fever was found to have West Nile neuroinvasive disease, so it is important to include West Nile disease in the differential diagnosis. Case Description: This is a case of a 45-year-old female with history of diabetes, hypothyroidism and recently diagnosed uterine leiomyosarcoma status post tumor debulking with metastasis on palliative chemotherapy with gemcitabine that presented to the Emergency Room for a fever of 103.8 degrees Fahrenheit. Given the history of advanced leiomyosarcoma, the patient was admitted for neutropenic fever with an absolute neutrophil count of 1000. During the hospitalization, the patient became acutely altered and confused. CT head without contrast and lumbar puncture were performed. Due to clinical suspicion of meningitis, she was started on broad spectrum antibiotics. Lumbar puncture revealed leukocytosis of 168 with lymphocytic predominance and elevated protein level in the cerebrospinal fluid, therefore acyclovir was started due to high suspicion of viral meningoencephalitis. An EEG showed severe diffuse encephalopathy as the patient was persistently altered. A broad workup of infectious etiology was considered including HIV, syphilis, hepatitis A, B, C, COVID-19, adenovirus, pertussis, influenza, WNV, HHV6, coccidiomycosis, aspergillus, and tuberculosis. Patient was ultimately found to have elevated IgM and IgG titers for West Nile Virus. Discussion(s): It is important to consider a broad spectrum of diagnosis in patients with metastatic carcinoma presenting with new-onset fever and acute encephalopathy. This includes working up for other causes of altered mental status including cardiac, neurologic, psychiatric, endocrine, metabolic, electrolyte, drug, and infectious etiology. While uncommon in the healthy population, WNV encephalitis should be on the radar for any patient who is immunocompromised or on immunosuppressive therapy, especially those who present with a neutropenic fever.

Objectives/Scope: Kuwait Energy is exploring, developing, and operating four concessions located in the Western Desert and the Gulf of Suez in Egypt;the company implemented many projects that had a significant impact on saving operating expenses and reducing greenhouse emissions to preserve the environment. One of these recent executed projects was replacing scattered diesel generators with a Central gas-driven electric power grid in Al- Jahraa field in East Abu-Sennan concession. In this , we present the challenges we faced during the planning phase and execution strategy applied to overcome these challenges. Methods: Al-Jahraa Field includes 13 running wells, a waterflood station, and a main oil and gas production plant. The field electricity is supplied by 15 scattered diesel generators for wells and facilities, consuming 100,000 liters of diesel per month. During the feasibility study phase of the project, many challenges were faced which had a negative impact on the project's economical assessment and that would result in cancelling the project, the challenges were summarized as following;the existence of wells at long distances from the site of the proposed main power station, which would require extending long lengths of electric power cables at a high cost, also the expected delay in the implementation and commissioning of the project resulting from the long delivery time of materials, especially copper cables and main switchgear during the COVID-19 pandemic. Several scenarios were studied for connecting the wells to the power station: The first scenario was to connect all wells and field facilities directly to the main power station. In this case, the estimated power cable lengths required to be extended were 25,000 Mt, in addition to using two 1 MW generators, one in service and the other would be a standby generator to provide backup power during a repair or maintenance service. This option economic model showed negative NPV due to the high cost of cables and extended execution time. Therefore, this option was cancelled. The second alternative was to connect each group of wells to three power stations to be operated using three diesel generators of 500 kVA for each station, with three backup generators. But the implementation of this option would lead to saving the cost of copper cables by 50%, but the cost of purchasing generators would increase due to the increase in the number of stations accordingly, in addition to the increase in operating expenses resulting from the increase in fuel consumption and maintenance cost compared to the first option. The third alternative, in which the economics of the project proved to be the best, is to divide the wells into three groups. Each of the two remote groups of wells are connected to an electric distribution panel, and then the two panels are connected by a main cable to the main power station. Moreover, the project cost was reduced by 50% due to the implementation of the following innovative optimization approach: • Re-using ESP cables instead of copper cables optimized both cost and delivery time as these materials are pulled from ESP wells. • These cables are designed for harsh downhole conditions increases its durability and extends its lifetime. • Using step-up and step-down transformers enabled us to reduce cable sizing, which also reflected on the lower cost of the project and, accordingly, increased its feasibility to be constructed. • An Incremental development approach, was followed in the management and implementation of the project, led to the speed of project delivery, and reduced the project risks and uncertainties. Results: The project was completed and commissioned within the allocated budget and time frame, leading to: ◦ 100% reduction of diesel fuel consumption levels. ◦ +68% reduction in total emissions;emissions are reduced by 2.5tons per year on average. ◦ reduced operational costs for each kilowatt hour generated due to using associated gas as fuel and releasing 13 rental generators. ◦ With the replacement of 1 rental generators with just one, the amount of maintenance waste, such as batteries, used oil, oil filters, fuel filters, and so on, is significantly reduced. ◦ These projects showed positive economic indicators (+NPV), with less than 1 years of payback. Conclusion: From this project's planning, execution, and results, we can claim that if risk assessments, detailed scope of work, good resource and time management, and cost-effective choices were addressed carefully, shall result in outstanding performance. The design of a high-efficiency electrical power supply system and use of associated gas in power generation reduces levels of fuel consumption, GHG emissions, and operational costs. Power generation project is a repeated case performed in one of our own assets in Egypt due to positive results and are easily transferable to sister IOCs & NOCs. ABSTRACT

This paper examines how firms have transformed and executed IT-enabled remote work initiatives during the COVID-19 pandemic. After examining archival data on a sample of 100 firms in Spain, we discover three types of IT-enabled remote work firm's strategies: leader, agile, and survival. Leader companies have a competitive advantage over agile companies, which in turn have a competitive advantage over survival organizations. We find that firm size was crucial to executing remote work firm's initiatives as a leader or survival. The industry significantly affected the implementation of remote work firm's initiatives during the three pivotal periods in the telecommunications industry. © 2023

ABSTRACT

The recent global pandemic renewed the importance of telecommunication infrastructure, as many COVID responses (e.g., working from home, home-schooling, e-commerce) were challenged by the inequity of access to broadband services and its underlying network. This paper examines the geospatial footprint of the National Broadband Network (NBN) in relation to the demographics of Australia's cities and regions. Through data made available by the NBN, which describes the technologies used in its multi-technology mix platform, and published demographic data available via the Australian Bureau of Statistics (ABS), we explore the equity of access to broadband technologies across the nation. The results show a persistent urban-regional divide in the NBN. The paper also offers a brief assessment of the complex and contradictory ways policy responses attempt to mitigate the digital divide, including updates to the national infrastructure project plans. ABSTRACT

Purpose: This study examines the impact of digital corporate social responsibility (CSR) on social entrepreneurship, organizational resilience and competitive intelligence during the coronavirus disease 2019 (COVID-19) crisis. It also examines the impact of competitive intelligence on social entrepreneurship and organizational resilience. Design/methodology/approach: Data were collected from telecommunication companies in Jordan with a sample of 223 managers, using Smart-PLS for analysis and testing the research model and hypotheses. Findings: The results reveal a significant impact of digital CSR on social entrepreneurship. They show that digital CSR significantly impacts organizational resilience. The findings also indicate a significant role of digital CSR in competitive intelligence. This study shows that social entrepreneurship significantly impacts organizational resilience. The results also confirm the impact of competitive intelligence on social entrepreneurship. Finally, the results confirm that competitive intelligence significantly impacts organizational resilience. Originality/value: This study provides valuable academic and practical insights into digital CSR practices, social entrepreneurship and how to support organizational resilience during crises. © 2023, Emerald Publishing Limited.

To assess the effectiveness of remote clinical quality management of endovascular Aim care. The system of clinical quality management of medical care in myocardial infarction (MI) including the quality of remote control of endovascular care was developed and introduced into the health care system of the Moscow Region as a part of the comprehensive study in 2008-2020. The number of people under the study was 8375. The ground for assessing the effectiveness of remote clinical management in 2019-2020 was the health care system of megapolis. Based on the analysis of 2966 endovascular procedures protocols, the treatment tactics effectiveness of intraoperative decisions was studied after an emergency coronary angiography (ECA) had been performed by interventional cardiologists. The Methods system of remote clinical quality management of endovascular care included a complex of audiovisual communications, computer system processes, mentoring and the algorithm for making an intraoperative decision. The effectiveness of remote clinical quality management of endovascular care was investigated on the number of percutaneous coronary interventions (PCI) in MI, mortality of patients with MI in the Regional vascular center in 2019-2020. The T-criteria was used to assess the reliability. The material statistical processing was carried out in the Statistica 6.0 package calculating adequate statistical indicators and their reliability at p<=0.005. Ratio PCI/ECA in 2019, January-March 2020 counted up to 48.95%. In April-December 2020 it increased up to 71.6% (p<0.001). The frequency of performing Results PCI increased by 1.46 times (p<0.001). Hospital mortality from MI decreased during the following period 2019, April-December 2020 from 9.7% to 8.2% (p = 0.005). Remote clinical management based on telemedicine and mentoring process Conclusion technologies contributes to improving the quality of endovascular care in MI.Copyright © 2021 Angles. With the increase of remote working during and after the COVID-19 pandemic, the use of Virtual Private Networks (VPNs) around the world has nearly doubled. Therefore, measuring the traffic and security aspects of the VPN ecosystem is more important now than ever. VPN users rely on the security of VPN solutions, to protect private and corporate communication. Thus a good understanding of the security state of VPN servers is crucial. Moreover, properly detecting and characterizing VPN traffic remains challenging, since some VPN protocols use the same port number as web traffic and port-based traffic classification will not help. In this paper, we aim at detecting and characterizing VPN servers in the wild, which facilitates detecting the VPN traffic. To this end, we perform Internet-wide active measurements to find VPN servers in the wild, and analyze their cryptographic certificates, vulnerabilities, locations, and fingerprints. We find 9.8M VPN servers distributed around the world using OpenVPN, SSTP, PPTP, and IPsec, and analyze their vulnerability. We find SSTP to be the most vulnerable protocol with more than 90% of detected servers being vulnerable to TLS downgrade attacks. Out of all the servers that respond to our VPN probes, 2% also respond to HTTP probes and therefore are classified as Web servers. Finally, we use our list of VPN servers to identify VPN traffic in a large European ISP and observe that 2.6% of all traffic is related to these VPN servers. © 2023, The Author(s), under exclusive license to Springer Nature Switzerland AG.

Objective: The relevance of the study is to primarily determine the predictors of adherence (distance, socio-economic status, disease status, and treatment schedule) of patients with dystonia, spasticity, and hemifacial spasms in a single center Botulinum neurotoxin injection (BoNT) study. Background(s): In December 2019, there was a new type of coronavirus identified in Wuhan, China which rapidly escalated to all parts of the world, the Philippines included. The COVID 19 pandemic did not only affect the patients contracted with the disease, but also those who have non-communicable diseases including dystonia, spasticity, and hemifacial spasm. BoNT is a chemodenervating agent that reduces muscle hyperactivity and spasms. Continuation of outpatient treatment may be affected due to the following reasons: lack of access to the health care system because of travel restrictions or lockdowns;fear of contracting the virus;and finances, which has a bigger impact on lower income patients. Method(s): This study utilized a cross-sectional research design with a total of 120 samples. Inclusion and exclusion criteria were met. Consent was secured. Data was gathered through chart reviews, telecommunications, and evaluation after receiving BoNT treatment in three different timelines-a year before (March 1, 2019 to February 29, 2020), during (March 1, 2020 to February 28, 2021), and late COVID 19 pandemic (March 1, 2021 to February 28, 2022). Result(s): The result of this study using percentages and p-values demonstrated a higher percentage in the distance and socio-economic status, as well as in the lost to follow up rate when compared to the other group. Distance and patients lost to follow up had p-values of less than 0.05. Therefore, only distance was noted to be a significant factor to the adherence status of patients. Furthermore, the lost to follow up rate in the COVID 19 pandemic period is also significantly higher when compared to that of the other group. Conclusion(s): It is concluded that patients with closer distance to the facility had better adherence hence the researchers recommend having an affiliate clinic or a referral facility for patients residing outside Metro Manila for better adherence.

The coronavirus disease 19 (COVID-19) pandemic had a profound impact on blood services operations in Korea. Blood collection was affected due to decrease in donor availability caused by avoidance of public places, social distancing policies, and cancellation of blood drives. The negative impact on blood collection was more pronounced with the COVID-19 pandemic than with other outbreaks experienced previously such as the influenza (H1N1) outbreak or the Middle East respiratory virus (MERS) pandemic. To cope with the blood shortage, campaigns to appeal for blood donation, raise public awareness on the importance of blood donation and gain donor's confidence in safe blood donation were implemented using mass communication media such as TV and radio broadcasting as well as postings on various social media platforms. Upon Korean Red Cross Blood Services's (KRCBS) request, the Ministry of Health and Welfare (MoHW) approved the relaxation of the geographical restrictions regarding indigenous malaria thus enabling collection of more than 23,000 units of whole blood. To mitigate even a theoretical risk of transfusion-transmission of SARS-CoV-2 via blood donation from pre-symptomatic COVID-19 donors, the KRCBS received the data on COVID-19 identified cases from the Korean Disease Control and Prevention Agency (KDCA) from the early get-go of the pandemic for cross referencing to donors for further recipient investigation and recall of blood products not transfused. Communication with donors, staff members, national health authorities, hospital customers and other stakeholders was and remains of utmost importance to respond to this unprecedented situation which is still ongoing.Copyright © Annals of Blood. The pandemic situation due to covid-19 has disrupted routine activities such as attending classes physically in educational institutions, which insisted on moving towards online education with the help of advent and increased uses of new telecommunication services. Service quality is the prerequisite for customer satisfaction. Service quality assessment is crucial to ensure increased customer satisfaction in any service. Typically, it is not easy to evaluate service quality because of opacity in the information, and incompleteness characteristics of problems. With the collected data through an online survey, this study aims to analyze the facts that influence the students' perception regarding the impact of telecommunication service quality on online education during the pandemic situation. Initially, some relevant criteria are derived from literature reviews. The proposed model is exerted to evaluate the quality of the online education and telecommunication service in Bangladesh during the covid-19 pandemic with the participation of 350 students answering 39 questions. The collected data is analyzed to assess the current state of service quality by evaluating the students' satisfaction using the entropy technique. The findings of the study suggest that the online education system in Bangladesh is not interactive enough, and the telecommunication service quality here is not sufficient for this purpose. Telecommunication challenges such as poor network quality, overpricing structure of telecommunication services and slow connection speed must be resolved to ensure satisfactory quality of online education.

The pandemics such as COVID-19 are worldwide health risks and result in catastrophic impacts on the global economy. To prevent the spread of pandemics, it is critical to trace the contacts between people to identify the infection chain. Nevertheless, the privacy concern is a great challenge to contact tracing. Moreover, existing contact tracing apps cannot obtain the macro-level infection risk information, e.g., the hotspots where the infection occurs, which, however, is critical to optimize healthcare planning to better control and prevent the outbreak of pandemics. In this paper, we develop a novel privacy-preserved pandemic tracing system, PRISC, to compute the infection risk through cellular-enabled IoT devices. In the PRISC system, there are three parties: a mobile network operator, a social network provider, and the health department. The physical contact records between users are obtained by the mobile network operator from the users’cellular-enabled IoT devices. The social contacts are obtained by the social network provider, while the health department has the records of pandemic patients. The three parties work together to compute a heatmap of pandemic infection risk in a region, while fully protecting the data privacy of each other. The heatmap provides both macro and micro level infection risk information to help control pandemics. The experiment results indicate that PRISC can compute an infection risk score within a couple of seconds and a few mega-bytes (MBs) communication cost, for datasets with 100,000 users. IEEE

In such a setting, technology can play a facilitating role, particularly in reaching out to remote locations and offering a greater standard of care at a lower cost. The method of treating and diagnosing patients remotely through communication networks is known as telemedicine. When more patients get access to telemedicine, payers take more notice of how much less expensive it is than traditional medicine, and doctors are aware of its benefits. Telemedicine is a more beneficial technology that can expand access to preventive treatment and may lead to long-term health. Telemedicine has the potential to greatly affect public health. This paper reviews the current state of the art of telemedicine in India. Nearly 50 years ago, telemedicine was shrugged off as a complicated, expensive, and inefficient technology. Because of how quickly the information technology and telecommunications disciplines are advancing, telemedicine is today a viable, dependable, and useful technique. Practitioners and medical experts from a variety of fields have experienced success with telemedicine. The COVID-19 pandemic highlighted the need for strong primary healthcare networks for a more effective public health response during health emergencies and exposed the fragmentation of healthcare delivery systems. Although primary care is the first point of contact between the general public and the healthcare system, it has not recently grown much focus or funding. Even in the post-COVID-19 environment, telemedicine offers the potential to get through enduring barriers to primary care in India, such as a shortage of qualified medical professionals, issues with access, and the cost of in-person care. Telemedicine has the power to speed up the delivery of universal health coverage while strengthening primary care. There is a widening gap between people and those who offer basic health services as the population in India has grown, and the average lifespan has increased. Telemedicine helps with palliative care, early identification, a better cure, prevention, and rehabilitation in the treatment of cancer. Due to a shortage of primary care delivery networks and referral units, secondary and tertiary care facilities' health systems are overworked. To successfully use telemedicine, proper planning and operating processes are required. Thus, the development and implementation of telemedicine will improve patient care and India's primary healthcare system in the future. Finally, telemedicine's cost-effectiveness will likely be its most significant outcome.

ABSTRACT

Covid-19 infection is an additional burden to the life of the Rohingya children living in cramped camps of Cox's Bazar, Bangladesh. BRAC has introduced Humanitarian Play Lab (HPL) for children's playful learning in the camps since 2017. During the Covid-19 pandemic, the modality was changed from face-face interactions to a telecommunication model. This qualitative research aims to understand caregivers' and frontline providers' practices and perceptions about children's well-being during the pandemic. Interviews were conducted with purposively selected parents and frontline providers through telephone. Interviews were transcribed and thematically analysed. The lockdown directly and indirectly affected children's mental and physical well-being. A shared parenting role was observed in child education and learning. Parents widely accepted tele-communication services for children as it was considered important for continuing children's wellbeing and learning. This research highlights the relevance and timeliness of utilising telecommunications services by parents for children's psychosocial health and playful learning. The NTT Group is participating in the international standardization activities in the International Telecommunication Union - Telecommunication Standardization Sector (ITU-T) Study Group (SG) 5 to protect telecommunication facilities from electromagnetic interference and lightning surges, assess the impact of information and communication technologies on climate change, address the issue of a circular economy that enables sustainable development, and contribute to improving the reliability of telecommunication services and reducing the environmental impact of its business activities.
